How to unlock the Third Time's a Charm achievement in Hogwarts Legacy - Definitive Guide
During the main story quest "The Elf, The Nab-Sack, and the Loom" you'll be introduced to the "Nab-Sack" for capturing beasts, a Vivarium to house them and how caring for them will provide you with materials. At the very end of the quest Deek will give you the Spellcraft for an "Enchanted Loom." The Loom will grant you the ability to upgrade your "Gear" using the materials collected from your cared for beasts.
The materials needed to upgrade the gear varies and the rarer the gear the more materials you'll need to upgrade it to the next stage. For example a "Common" set of Gloves may require 1x Jobberknoll Feather, where as an "Extraordinary" set of Gloves might need 3x Jobberknoll Feathers. Also reach rank of the upgrade tends to require an additional material from another beast. So upgrade one might need 1x Mooncalf Fur but upgrade two will need 2x Mooncalf Fur and 1x Puffskein Fur to upgrade.
As long as you have captured a good variety of beasts and collect their materials regularly it should be pretty straight forward to upgrade an item to level 3. If you wanted to get this achievement out the way, pop a "Common" item into the "Enchanted Loom" and see what beast material is needed. If you don't have said material, head over to "Brood & Peck" the beast shop in Hogsmeade and you can buy the materials needed instead.
Note: Each beast will give you 3x/5x their material every 25 minutes when fed and bushed.

Groom and feed the animals to get the resources needed to upgrade a piece of equipment (blue being the easiest) three times. Check on the loom to see what item you will need to buy from the vendor and then travel to the vendor to buy it and then finish the third tier of upgrade. (all shown in video)

Once you have unlocked your magical loom within the Room of Requirement, simply fully upgrade a piece of gear using materials you have farmed from your tamed beasts to unlock this achievement.
Note: I disregarded the loom for the majority of my play through, however the upgrades and higher level traits can help boost your damage output significantly.

In Hogwarts Legacy there will come a point where you’ll actually be able to upgrade your pieces of gear, whether that’s your hat, gloves, cloak, or whatever. However, this isn’t something you get access to until quite far in the game. In fact, it’s not until you complete the 25th main mission (out of 40) “The Elf, the Nab-Sack and the Loom”, where you’ll get access to the Enchanted Loom. This valuable item in your Room of Requirement will not only allow you to apply traits to your gear, but also upgrade its stats too.
To upgrade its stats though, you’re going to need to build a bit of a farming system in your Vivariums, so I refer to The Nature of the Beast (30G) achievement for details on where to get what specific beasts in order to unlock the necessary resources you’re going to need.

Each item has 3 upgrade slots. You need to upgrade any one item to level 3. I advise you to do this at level 40, on 40 legendary type items. The reagents required there are rare.

I advise you to do it at level 40 where rare reagents are like that, as you will open the opportunity to update/upgrade your clothes, take any item with 3 blue/green holes (not sure about the green ones) and pump up 3 times, they have the cheapest reagents

Let me add that for the sake of the trophy, you can pump up all 3 cells as soon as the opportunity arises. Fortunately, all the ingredients can be bought in Hogsmeade from the animal trader
