How to unlock the Followed the Butterflies achievement in Hogwarts Legacy - Definitive Guide
I obtained this by completing the side quest: Follow the Butterflies. It can be obtained from Clementine Willardsey in the Three Broomsticks after finishing the main quest: Potions Class. Simply travel to the marker in the Forbidden Forest and follow the butterflies as they lead you to a chest. The achievement should pop once you have received the treasure inside of the chest.

I veered off to the spiders and the poacher camp and was still able to get the chest, so must have just been an unlucky glitch

I have followed the butterflies to the treasure in both the main story and in the open world but it won’t unlock, why is this?

Eventually you'll start encountering Butterflies and their distinct Butterfly icon on the map. Simply walk up to a Butterfly Icon and you'll be warmly greeted by a kaleidoscope of Butterflies. Follow the direction they take and try your best to stay as close as possible. After a couple of minutes walking, they'll eventually stop and reveal a hidden chest. Open the chest and the achievement should pop.
A side-quest for Clementine Willardsey will also appear in the "Three Broomsticks" pub once you have completed your first lesson in the Potion Class. During Clementine's quest you'll also need to follow Butterflies to a hidden chest.

This guide can be missable if you skip side quest. To get it you need to talk with Clementine Willardsey in Butterbeer Bar in Hogsmead village. I did it after the Troll attack. The quest is pretty straight forward. Someone said the quest can be bugged, so make sure to save it before talking with Clementine.
Butterflies can be found all over the open world of Hogwarts Legacy. Their location will be marked on your map when you get close enough to them. Additionally, there is a side quest, "Follow The Butterflies", that pops up early in the story after visiting Hogsmeade for the first time. Complete this side quest, which marks a butterfly location on your map and makes you follow them for the achievement / trophy!
While you can get this achievement for just following butterflies to some treasure - you’ll get them appear on the map whenever you go close to one - if you wait until after you’ve unlocked your Talents skill tree (or thereabouts), a side quest will pop up in Hogsmeade called “Follow the Butterflies.” Speak to Clementine and then follow the quest to its conclusion. This achievement will pop as soon as you find the treasure. Nice and simple.
After you've finished this side quest, there are still 14 to collect in the open-world, especially if you want the Collector's Edition (40G) achievement. We have a guide for all Hogwarts Legacy butterflies locations you can use for that.

When you find a group of butterflies while exploring the area outside Hogwarts, follow them until they come to a rest. They will reveal the hidden chest at that location.
These butterflies are located northeast of the Pitt-Upon-Ford in the far north part of the map.

The "Follow the Butterflies" quest didn't appear for me until I completed the "Jackdaw's Rest" quest. I suggest completing "Jackdaw's Rest" if the "Follow the Butterflies" quest doesn't show for you.
Then once you get the "Follow the Butterflies" quest, complete it to get this achievement (the achievement pops once you open the chest during the quest).

You need to complete the side quest “Follow the Butterflies” from Miss Willardsey, you can find her in Hogsmeade, in the Three Broomsticks pub.

If for some reason you missed the butterflies during the surveillance process (for example, you were attacked by an enemy), then an icon in the form of butterflies will appear on the map, where they will be waiting for you to continue their flight (or they will already be above the chest and you just you need to come to the mark and wander)

You just need to complete the quest and find one chest for the trophy; it is very convenient to follow the butterflies on the mini-map, since their display changes in real time.
