There are 5 Cairn Dungeons in total. 2 of which can be entered at any time, the other 3 can only be accessed via quests. I've listed all 5 Dungeons below with there location or required quest:
Dale Family Tomb: Hogwarts Valley > Brocburrow. Head east from the Brocburrow Floo Flame.
Tomb of Treachery: Poidsear Coast > Tomb of Treachery. Under the bridge at the very North of Poidsear Coast.
The Collector’s Cave: Entered during the main story quest “The Helm of Urtkot.”
Feldcroft Catacomb: Entered during Sebastian Sallow’s relationship quest “In the Shadow of Time.”
Moonstone Garden: Entered during Poppy Sweeting’s relationship quest “The Centaur and the Stone.”
Note: Each dungeon needs to be entered to progress the achievement. Simply discovering the location isn't enough.

Hey, one thing, the Moonstone Garden is unlocked during "The Centaur and the Stone" quest, not the "Fire and Vice". I think Fire and vice is the one prior to the Centaur one!

This is technically unmissable - if you follow the main quest and all side quests you will get this trophy.
Cairn Dungeon 1: The Collectors Cave
You will get this dungeon by completing ‘The Helm of Urtkot’ main quest. It can be found in the forbidden forest west of Hogsmeade.
Cairn Dungeon 2: The Tomb of Treachery
You will get this dungeon by completing the ‘Cursed Tomb Treasure’ side quest. To unlock this side quest you must find a map fragment hidden in a locked chest under the old manor at Manor Glen. This map fragment will lead you to the tomb of treachery. This cairn dungeon can be found as you exit the goblin mine and is hidden behind a waterfall.
Cairn Dungeon 3: Dale Family Tomb
You will get this dungeon by completing the ‘Beeting a Curse’ side quest. It will be hidden on the map until this quest is unlocked. To start this quest you must speak to Samantha Dale outside the Herbology greenhouses in Hogwarts. This dungeon can be found to the far east of Hogwarts in the Highlands, the closest Hamlet and floo flame location is in Brocburrow.

You will get this dungeon by completing the ‘In The Shadow of Time’ relationship quest. You will receive this quest following an owl sent by Sebastian regarding a lead on a relic he believes will help his sister. It is found south of the Feldcroft hamlet.

You will get this dungeon by completing ‘The Centaur and The Stone’ relationship quest. You will receive this quest following an owl sent by Poppy requesting you meet near Irondale. This dungeon is inaccessible prior to completing this quest. The dungeon itself is slightly west of the Irondale hamlet.

For this achievement you need to discover, i.e. step foot into, all of the cairn dungeons dotted around the game world. You'll know what is and isn't a cairn dungeon by the outside of the dungeon itself. If there's a ton of rocks pilled up, like stonehenge, that's a cairn dungeon.

To get a trophy, you need to not only discover, but also enter each one (it is not necessary to go through completely).
You can get to the burial grounds without any quests by simply finding them on the map, but you won’t be able to get through them to the end without quests.
The trophy will drop automatically when you complete tests for the main and side quests, since each burial ground has a quest associated with it.
