This achievement requires a comprehensive playthrough of the game where you basically do everything. The game shipped on PC in 2017 with 100% potential progress and content was added that increased that total to 112%. The Xbox version contains all of the content from the start.
After destroying the three Dreamers you can get into the Temple of the Black Egg in the Forgotten Crossroads. Interact with the statue next to the bench and your completion percentage will then be shown in your menu and on your save slot.
For progress towards 112%, this is what counts:
Upgrades/Pickups (38% total):
Charms (1% each charm = 40% total):
Bosses (1% each = 17% total):
Warrior's Graves (1% each = 7% total):
Dreamers (1% each = 3% total)
Colosseum of Fools (1% each = 3% total
Godhome Pantheons (1% each = 4% total0
Once you reach 112% progress, complete the game and the achievement will unlock during the post-credits summary screen.
The Hollow Knight wiki is a great source of information on anything specific.

thandk for the followup, one more question, are you certain the pantheons are only worth 4%? is it possible it is worth 1.2% or something similar?
i am currently sitting at 110% with only the 4th pantheon remaining.
i have double and triple checked all the other requirements.
EDIT:just beat pantheon of the Knight and I am indeed stuck at 111%. ONE noticeably odd thing I’m finding is I am missing 3 entries in the hall of gods . One below mantis lords,one below nosk and one below nightmare king. I have gotten the hunters mark, all upgrades , completed all endings except the final pantheon, all colloseums and all warrior graves. Pretty much the only thing I didn’t get is menderbug.
EDIT 2:I figured out I was missing the sisters of battle and winged Norsk which apparently are unlocked after entering the 5th pantheon. Radiance should’ve shown up he was my last missing boss but I’m not certain why he didnt
Also I still have one of the breakable charms, think I will start there.
EDIT3: I was missing abyss shriek, 112% attained.

thanks for the breakdown , all other sources ive seen were not nearly as concise or reliable i would argue.
I have yet to clear the game but am probably approaching 107% right now, are you aware whether completeing the game a first time will affect this 112% achievement at all or does it need to be the first time beating the end boss?
