I believe that this is unobbtainable.
Seeing as there are only 4 types of enemies in the game it is not kill all 4 types of enemies.
I did try however play through the whole game and kill everything that came at me. I backtracked like crazy and even killed all the pukers. It is possible that I missed something but seeing as this game has 7 other achievments that noone has, I am of the belief that there is something wrong with this one and not in a good way.
