Without giving away spoilers this achievement is earned by completing the final sequence of the game.
Just to note. This is technically unmissable however I completed the game andachievement did not unlock straight away. I had to delete the cache (hold power button for 10 seconds) . The achievement popped randomly 24hours later.

This achievement glitched for me as well. Deleting the cache did not work in my case. However, after finishing the game I noticed that if I chose to load from the main menu, that it had a few backup saves available. The oldest one listed was right before the point of no return. I was able to load in and start the endgame stages over, unlocking the achievement after the first section. Right after the cutscene where you are looking through the TV camera is where it should unlock.
So if you finish the game, and it hasn't unlocked:
On the main menu, Do Not choose Resume
Go to Story
Select Homefront: The Revolution
Make sure to choose Load
Of the 4 most recent saves available, it should be the bottom one
As a side note, this should also work for anyone who had trouble unlocking the achievement for completing the campaign (Land of the Free) or the one for Deathwish difficulty (Home of the Brave). It might even be used as a workaround for returning to the open world section in case you missed anything. I hope this helps.

Story-related, cannot be missed.
This mission takes place after the point of no-return. Complete the mission "A Roll of the Dice" and you will get the trophy.
