You will find ghosts in the final section of the Miasma, the City. The first one you encounter is just down the hill at the start of the area past the "light". You will need to get your health to one "heart", though I ensured it was slightly less just in case. The Ghost does about 4 damage, while the light does about .75 damage and further down the hill are spikes that do 2 damage.
Once down to or below 1 "heart" you have to repel the ghost with a Flare Potion. Let the ghost see you, then when the indicator above it’s head is full and it starts chasing you, use the potion to repel it.
You can save right before entering the Miasma and turn off autosaves in case you mess up or just don't want to waste a potion.
The recipe for the Flare Potion you will find further in on the left of the next two Ghosts you'll see. To craft it you'll need Beast's Blood Resin, crafted at the mill with Beast's Blood flowers found in the Ancient Swamp and Rhubarb, the seedling can be found just past the first set of spikes in the City.

Note: you need to aggro the ghost before casting the flare spell!
The moment the aggro bar is full turn on the flare spell and you should see the aggro meter lessen and the achievement should trigger right away (assuming you have 1 health or less - I got to 1 health by starving)
I tried this numerous times before finally figuring out that the achievement was not triggering for me because I was casting the spell before approaching the enemy not providing a chance for them to aggro.
