You just need to complete New Game+ on any difficulty.

You don't need to focus on any side quests, just go straight for the 22 main story quests.

Took me around 5-6 hours to do!

If you want the other New Game+ trophy, you will need to complete this on Ultra Hard difficulty.

This trophy will pop upon completing New Game+. If you do it on the new Ultra Hard difficulty, you will also unlock the Ultra Hard Completed trophy.
