Marty is a snail that will spawn in-game after the player has died on at least 10 attempts. Once you have, you can visit the piano room on the right-side of the house, where he will spawn. Speak to him and say "yes" when he asks if you would like help.
It's worth noting that Marty is also required for the achievement "co-dependent", where you need to complete the endings with his help. He will disappear and re-spawn back in the piano room each time you die, so if you are attempting to complete the endings with him, you'll need to go back and grab him again. He can only be found on Tabby's playthrough of the game as well.
Lastly, if you pick up Marty and then kill one of your family members, the next time you spawn, Marty will be unhappy with you. You'll need finish off that run (either complete it or die) in order to pick up Marty again.
