You need to get all 206 game girls and complete the final mission for the "true ending" in order to unlock this achievement.

There are guides online for the stream version that help with this.

I did this and it didnt trigger. Wtf... The only thing I can think of is that since I collected all gamegirls on the first playthrough that I might have to replay the end levels once more. I hope that works....

The achievements in this game are bugged out something crazy. So many people have completed the requirements to a T and still haven't had them pop. There is a forum about it on this site somewhere where they even reached out to the devs and they haven't bothered patching it. It sucks
