You can get this quite easily: After completing all the quest for Kovac, you can free roam he's island. Go back to the workbench where you crafted you're first gun. There will be a walkie - talkie that has a quest. Just accept it and go to right most beach of the island (just simply go to right all the time). There you will find a boat that will take you to the Horno Island.

Once you've reached Kovac's island (La Vendida) and completed his tasks you'll be able to take boat from the southeast dock on the island. Take the boat and the achievement will unlock.
