At Bloodline Rank 45, you have access to a trait called 'Tomahawk' which allows you to throw hammers and axes picked up in match. It costs 2 upgrade points. You can also recruit a hunter that might randomly have this trait even if you don't have it unlocked yet. Like other thrown items, pull left trigger to aim and right trigger to throw.
SKILL-BASED METHOD (aka LUCK-BASED METHOD)
According to the description, it has to specifically be a sledgehammer, so pick up one of the numerous sledgehammers in and around buildings and when you find another player, throw the hammer at them. Depending on the other players health, this may not be an instakill, so prepare yourself for disappointment if you manage to pull this off legitimately in a match and the other player takes the sledgehammer to the back of the head like a champ and proceeds to turn around and blow you away with a shotgun.
BOOSTING METHOD (aka THE METHOD FOR MOST OF US)
This can easily be boosted by searching for a match at the same time as your boosting partner and coordinating a location to meet on the map, preferably at the edge of the map away from potential PvP hotspots. The server population seems to be low enough and the matches long enough that finding each other hasn't been an issue. Once you load in, just confirm both of you have the same map, the same targets, and the same time of day and you're probably in the same match. In our experience, it takes two thrown hammers to get a kill when the target player has full health and no special traits equipped that might mitigate some of the damage.

Is this achievement sill unobtainable? I'm asking since the new uddate 1.3 is online for xbox.

Boost it with mates, you can get into the same servers with two teams of duos.
Key point - buy a legendary hunter to try to roll the Tomahawk trait or bring a hunter you’ve upgraded previously with the purchased trait.
Make sure you select the same geographical server region(s) - do this by clicking on the globe...
e.g. US West and each party leader counts down to when to start the server search.
You can confirm over voice party if you are in the same server, based on map, number of contracts, which contracts, time of day and finally how many and where the extraction points are on the map.
Next thing is to meet up on the map, best idea is probably at an extraction as it’s less likely to have other enemies hanging out there at the start of the game(!) and make sure you come up with a way to identify each other, maybe all choose the same legendary hunter skin? Or do a funny dance move upon greeting each other!
On the way to meeting your ‘tomahawk’ squad, search for a Sledgehammer, search buildings and clue sites.
Then, decide who is going first and line up and throw sledge! If you all need to get the achievement, then you can revive your team mate and swap over, so team A gets hit first, revives and team B gets bit second and so forth, until all four members have the achievement and presto!
Seriously, I’ve tried this the legit way, it’s nigh on impossible or super lucky, as a hunter with full health is very unlikely to get hit once by the sledge and die, so you either have to trap up a door way and watch it, primed with the sledge and have a lot of patience or come up with some inventive method to trick your enemy hunters, who by now probably are not that green to stare at you long enough for a hammer to take their cowboy hat off...

Start alone against the teams, there will be more rustling, there will be more fuss, it’s easier to go unnoticed, take damaging tripwires (just in case). The perk is called INSTITUTOR . We launch with a friend at the same time as the NECROMAN T perk, or with two friends who will be on the same team against us.
