Hunted: The Demons Forge Walkthrough Page 4 - Chapter 2
The achievement is mainly focused on finding the main crystals you obtain from the containers (credit to Phoenix C64), NOT the ones you form from shards, though you are still encouraged to collect shards as well. There is 70 crystal containers throughout the game, so if you have been collecting these crystals respectfully; at the end of the game you should have about 132 crystals give or take a few just to have an idea where you are at. Please don't think because you have 132 the achievement will pop, as the shards you are collecting are added to the same total, so there will be people who have less or more and with no achievement.
To help keep track of your progress, here is the total of crystals you can find in each Chapter. (Credit to the TA Collectibles Guide and those involved):
Prologue - 3
Chapter 1 - 10
Chapter 2 - 14
Chapter 3 - 14
Chapter 4 - 7
Chapter 5 - 11
Chapter 6 - 11
Total - 70
Be careful of starting a new Adventure+ game as it wipes your collectibles progress. If you do happen to start a Adventure+ keep in mind you'll start out with 30 extra crystals so your ending total will be more than the guesstimated 132, simply look for the 70 containers for your crystal count.
Also if you play the game in local split screen, it seems the main person will only receive the credit, i.e. you have a buddy join you on second controller, they will not receive the credit for collected crystals.
You can check your progress while in game by pressing the BACK button and RB once for your Abilities, a list total should appear. After finishing a chapter or even the game you can replay sections to find the ones you didn't get. If you stick to the guides and find the crystal containers in their respective locations you should be on the right track.
If you feel you still aren't getting the achievement like you should, then please check out LosersVoteParty solution as another alternative for the solution. Any helpful information even from another source is always welcomed.

This solution is wrong. You say "There is a total of 132 crystals to be found throughout the game inside the cylinder caged containers. " yet in the game there is only 72 caged crystals. according to the official guide

Reading through their guide again, I need to fix that, thanks. Changing it to 132 total, through whole crystals and crystal fragments which complete said whole crystals. To be honest though, the Prima Strat Guide for Demon's Forge is inaccurate as well with totals and locations as IDiivil has pointed out.

Reader beware. Although the walkthrough correctly lists all full crystals, there is info in RaajTM's guide that is incorrect and may lead you to not unlocking the achievement. Crystal fragments are essential to get this achievement. Don't fall into that trap.
Full Crystals
There are two different types of crystals. One is full crystals that are located in specific locations throughout the game. There are 70. Here is a link to the TA Walkthrough:

Crystal Fragments
The other way to get a crystal is to pick up crystal fragments. Crystal = 5 fragments. There is no possible location guide for these fragments because they are somewhat random. Crystal fragments are:
1. Found in destructible vases that are teal and black.
2. Dropped by fallen enemies.
The only solution is to seek out these crystals whenever possible. My strategy was to follow my own pattern of destruction. I would kill an enemy and immediately run to see what was dropped while killing the next enemy. This way any dropped fragments are less likely to disappear. Earlier I said they are somewhat random. This is because you can't farm one area of enemies past a certain point. It is likely that you can only collect a limited number of crystal fragments per sub-chapter.
Total Crystal Requirement
There is an in-game counter for the crystals. Press select and then RB to see the total. You will need 132 crystals (162 if playing New Game+ because it gives you 30 to start with).
70 Full Crystals
+ 62 Crystals Created from Fragments
132 Total Crystals
Buggy Achievement
If only it were that simple. This achievement is listed as buggy for a reason. Many people have reported (myself included) that they are sure that they collected each full crystal and even got 132 crystals and the achievement did not pop.
My Theory:
You must have a minimum of 131 crystals (161 for New Game+) prior to picking up your last full crystal (wherever that may be).
This means that you must create at least 62 crystals from fragments before you pick up the 70th full crystal. That 70th full crystal can be it last in the game or any that you may have missed along the way.
In my first playthrough I collected my 132nd crystal from fragment creation after collecting my 70th full crystal. No achievement I wanted to make sure I hadn't made an error by missing a full crystal so I replayed the full game a second time. In doing so I not only checked each location, but I also verified that there was a ghost of the crystal in each canister (It is possible to open the canister and not pick up the crystal). I made no error. So I played the game in New Game+ (which resets your progress) and focused on fragment collection as well as full crystal collection. Achievement unlocked There were no additional crystals that I had not picked up the first time.

I think if you can't pick them up after they have dropped it means that you have collected all you can for that area. And I really don't know about you last question. Perhaps you can count the guide you are using to see how many full crystals you have picked up. Then subtract it from your total crystals. That way you can know how many crystals you have created from fragments.

If I run over crystal fragments and I don't pick them up, does that mean I have them all already? Stupid question yes but I just want to make sure it's not a glitch or something. When I collect all 62, will they still drop or will they stop dropping? Thanks :)

This is a quick (possible) solution to anybody who's looking down the barrel at a new playthrough after hitting a glitch with this achievement.
I had no issues unlocking this achievement on the standard version of the game (PAL/NTSC), but had the glitch occur while playing through the Japanese NTSC-J copy.
After going through each chapter and checking every crystal again, I confirmed I had collected all full crystals from the containers - which meant that I was going to have to retry this achievement on a new playthrough.
So from the experience of unlocking the achievement without hassle the first time, then hitting the glitch on my second time round - I thought I'd let people know how I eventually got it to unlock for me.
On my first (PAL) playthrough, I unfortunately missed these two crystals in Chapter 4:
Crystal (2-3/7): Now go through the hole that the Minotaur made and take a left. Talk to the fireplace, collect the 4 bones and light the green fire with Elara's arrow. Go up the stairs, alight the fireplace from the other side (through the broken floor) and enter the now open area for the crystals.
I continued to play through the rest of the game (collecting all shards from enemies as well), and after beating the last boss, I loaded up Chapter 4, grabbed the two I missed and unlocked the achievement.
When playing through the Japanese version, I didn't miss a single crystal, grabbed all shards from enemies, and when I got to the last 2 full crystals in the game - the achievement didn't unlock.
After double-checking all crystal containers by replaying chapters, I resigned myself to another playthrough just for this.
I started a New Game+, and played through the game again.
HOWEVER, this time - I missed those two crystals on Chapter 4 AGAIN. My idea was to replicate how I got the achievement to unlock the first time. After beating the game (getting the rest of the crystals and collecting all loose shards), I loaded up Chapter 4, got the crystals and unlocked the achievement.
So - my point is... if you have to replay the game because of the glitch, it might be in your best interest to miss one or two, then collect them after beating the game. I know it worked for me twice.

For what it's worth, I went through the J version (as you know) and didn't miss any crystals, it popped at the last one. Knowing this game though, I could have just been super lucky...
