This achievement can be earned in Flying Fish Level 2: Into The Depths.
First you will need to get to the large open cave area with minecarts and progress far enough until you have raised the water level to maximum height.
Now return to the area with the minecart on its side which hides a piece of bread. It is also the area where you will have been blasted by a water pipe into a minecart full of water which is on some tracks suspended in the air.
In this area you will see the minecart track floating in the air (water) which has been broken so there is a gap, one side leading to the minecart which is now in front of a gate, and the other side leading into a collapsed tunnel.
Swim so that you are directly facing the collapsed tunnel. Now look up and to the left (roughly 45 degrees up and 45 degrees left). Right at the surface of the water you should see a triangular gap in the wall which you can swim through to earn the achievement and find what has crashed in the cave.

Thanks for posting this. The quick way to get to this point is to start the level, press Pause and "Next Checkpoint" 10 times. From that point, turn Right, Right and Right again. This will lead you to the broken rail. Up and to the left is the little triangle you swim into. Here is a picture: https://i.imgur.com/o84E0Tk.png

Someone else posted a great guide, but here's a visual walkthrough starting from when you raise the water the second time, which is required.
1. Raise the water twice.
2. Go backwards in the mission until you get to the spot where you were shot up in the air, into a minecart filled with water for the first time.
3. The tracks ended in the middle of the open cavern, short of a caved in tunnel.
4. Face the caved in tunnel, and you'll see a small hole in the wall up and to the left of the tunnel, by the edge of the surface of the water.
5. Swim/jump in to find the secret area.
