How to unlock the Another Reality achievement in Immortality - Definitive Guide
UPDATE 9/8/22
After a game update on Xbox on 9/7, it looks like this achievement is finally unlocking for folks! It'll take time before we get a definitive method for getting to unlock, but in the meantime, the following method seems to be working for a lot of people:
Make sure your game has been updated. You can check the file info on your Xbox and it should have the words 'Updated 9/7/22' or whichever day you updated. After you've done that, fire up the game. Hit Continue. Then, proceed to re-watch EVERY subverted clip in your Minsky clips. It seems several subverted clips have migrated or disappeared from some of their original locations. Watching through all of them hopefully re-logs the missing ones for your achievement tracker.
After you're done doing that, save and quit the game. Then, head to the Xbox dashboard and force close the game. Re-open it, wait for the main menu to load, and hit continue. Hopefully, if you've discovered all the clips, the achievement will unlock. If, like me, you had more clips to discover, you'll have to go through your other clips one by one to discover new subverted clips. For me, I needed one new subverted clip in Minsky. Best of luck!

The community over on the forum has done a fantastic job compiling all of the known subverted footage clips. Credit to user The Cherished for opening the thread and compiling the opening post with known subverted footage clips.
To explain the methodology for finding these clips: typically what I do, once I've unlocked a clip, is run it all the way to the end (by clicking right thumb stick), and then slowly scroll backwards (using left thumbstick) to see if there's any hidden footage. You should be able to identify hidden footage by audio clues (a kind of foreboding tone) or by intense vibration on the controller. There are two types of hidden footage, typically: black and white scenes "underneath" the normal footage, and color scenes which are "overlain" on top of the original footage.
If you have a black and white scene, typically you'll have to "adjust tracking" on the film while scrolling backwards in order to align it properly to get the scene to play. You do this by adjusting what direction your pointing the left thumbstick, anywhere from "12 o'clock" to "6 o'clock". Once you find the right spot, wiggle it a little bit and eventually the scene will slot in and play properly. You might have to roll the footage forward a couple times and then rewind again to make it work.
If you have a color scene, no tracking adjustment necessary. Just keep rolling the film back slowly and the scene will eventually start playing over the 'normal' scene.
Keep in mind there are also clips with multiple subverted footage clips. Typically these are 'nested' inside other subverted footage clips. So, if you roll footage backwards and find a hidden clip, sometimes after you reach the end of that hidden clip, you roll the footage FORWARDS inside the hidden clip and find yet another clip. Some clips have as many as five hidden clips within them. Using this methodology to check every clip, you should be able to find them all.
And another note - some clips have the same subverted footage. It's unclear if you need to find ALL of the multiple copies of the subverted footage in order for the achievement to pop.

I was a little bit confused by this achievement. There are 3 sets of secret clips:
-15 secret clips that appear randomly somewhere. It's different for every player. (See the Secrets (2) section in the link for this list.)
-Ones that always appear in the same clip. It's the same for every player. (See the Secrets (1) section in the link.)
-A few secret clips that are hidden inside of other secret clips. Again, same for every player. (Secrets (1))

I'm not going to argue with you, your the one who is being difficult and being hard headed.
The person, obviously, was playing on PC with Gamepass, if you go to there profile you can see they play Xbox PC games, there's no other explanation other then that person hacked the game to get the achievement. From A Krayt Dragon's comment - "The person who unlocked it admitted they hacked the game to get the achievement unlocked. Developer has acknowledged it is currently unobtainable."
And if I was assuming stuff then how come other people who is stuck at 96% and is still playing the game to see if they missed anything hasn't unlocked it yet? Since you know everything.
The only reason why some people are less then 96% is because they probably haven't seen all the subverted footage all the way through to get to 96%.

There are some clips that are procedurally assigned and with some unlucky RNG it is possible to get a duplicate -- if this happens twice in Minsky, you run out of slots for fresh clips. Hence you not having all the clips and not unlocking the achievement. We're working on a patch that will fix the issue and also repair any saves for people who are already in this situation. Apologies!"

Just posting here to have all the updated information for this achievement in a more succinct manner.
Subverted footage can be discovered by rewinding when there's a notable tone/sound while viewing a clip. The controller will also vibrate.
When viewing the overview menu of all the clips there will also sometimes be a notable sound as well when near a clip that has subverted footage you haven't discovered. The sound will grow louder as you near the clip and be loudest when hovering over the correct clip.
(The clip might also have a more noticeable white hue around it)
Some of the subverted footage gets "locked in" to automatically play just by rewinding.
If rewinding only shows the faded overlay of the subverted footage, that's when you want to continue to slowly rewind then hold left on the dpad to go frame by frame "locking in" the reverse scene almost immediately.
Certain clips have multiple subverted scenes nested within each other. Do the opposite action to activate the next nested scene. (Rewind, then Fast Forward, Rewind, etc.) When having to fast forward to get the next nested clip and it gives an overlay it still was easier to hold left on the dpad to "lock it in" instead of right on the dpad which you'd think would work but doesn't.
Achievement tracking progress only updates when loading up the game. However, it will unlock normally while playing once you get 100% of the clips.

Excellent guide, thanks! I thought I would be annoyed having to go through all the subverted footage again to find what I'd missed, but actually I found it clarified the story for me quite a bit to see it all in sequence and helped to "complete" the game experience.

The game has removed some "Marissa" talking subverted footages in recent updates, but not all of them, so you still need to find them in the videos that are labeled "Marissa" talking in A Krayt Dragon's guide.
However, there is still a quick way to know which "Marissa" talking video contains subverted footage and which does not.
If your controller keeps vibrating every few seconds while you rewinding, it says there is no subverted in this footage, otherwise if your controller keeps still you should keep your mind.

Once you have collected enough pieces of the Film Grid, you may notice a different set of characters appearing on screen when you watch certain clips backwards. These are the Supernatural elements that are hidden in 51 designated scenes in the collection of footage. They feature a slicked-back short-haired blonde woman and a few other individuals replacing Marissa and other actors during scenes or interviews.
There are also 17 additional supernatural clips that show the blonde woman in a black and white scene, talking straight to the camera. These will be placed randomly in clips other than the 51 listed below. The best suggestion I can give to discover all 68 of the Supernatural clips, is to watch the clips listed below as well as every other clip of the 202 in the Film Grid from the end of clip to the start by scrubbing backwards until the extra 17 are found.
An important thing to note: A handful of these Supernatural Clips are a series of elements that will require changing direction to continue viewing the hidden footage. It is also important to make sure to use both the reverse scrubbing and using the finer detail backwards direction and vice/versa. For example, there are instances where the Supernatural Clip may not unlock fully if you only reverse with / so using will provide a more precise motion backwards which will pinpoint the unlock of the clip, then you can watch the unlocked clip fully with /. Most clips seem to prefer to be unlocked with only /. In the instance of multiple Supernatural clips back to back, it may be best to go between and to determine which direction each clip segment wants to unlock and then follow it up with / to view the next section of the unlocked clip.
With all the Supermatural clips, you will want to listen for an audio cue as well as keep vibration active on your DualSense controller during your gameplay as it will help a lot with discovery, especially if you are using a quicker reverse method to get through 202 clips. Each Supernatural clip should be visible long enough for you to catch if you are paying attention, even at the fastest scrub speed, but it would be a shame for you to miss a clip along your way.

As stated above, the best policy is to watch all footage not listed above backwards until you find them. See Immortality for a little more information on the last two "endgame" clips.
This task is the biggest barrier to the platinum. If you are still not seeing this trophy activate after finding the fifeteen Supernatural clips hidden randomly, viewed the end scenes of the game, and watched all of the 51 clips listed above, go back and rewatch the supernatural clips that have multiple segements to make sure you truly watched every clip.
There are also 17 additional supernatural clips that show the blonde woman in a black and white scene, talking straight to the camera. These will be placed randomly in clips other than the 51 listed below. The best suggestion I can give to discover all 68 of the Supernatural clips, is to watch the clips listed below as well as every other clip of the 202 in the Film Grid from the end of clip to the start by scrubbing backwards until the extra 17 are found.
An important thing to note: A handful of these Supernatural Clips are a series of elements that will require changing direction to continue viewing the hidden footage. It is also important to make sure to use both the reverse scrubbing and using the finer detail backwards direction and vice/versa. For example, there are instances where the Supernatural Clip may not unlock fully if you only reverse with / so using will provide a more precise motion backwards which will pinpoint the unlock of the clip, then you can watch the unlocked clip fully with /. Most clips seem to prefer to be unlocked with only /. In the instance of multiple Supernatural clips back to back, it may be best to go between and to determine which direction each clip segment wants to unlock and then follow it up with / to view the next section of the unlocked clip.
With all the Supermatural clips, you will want to listen for an audio cue as well as keep vibration active on your DualSense controller during your gameplay as it will help a lot with discovery, especially if you are using a quicker reverse method to get through 202 clips. Each Supernatural clip should be visible long enough for you to catch if you are paying attention, even at the fastest scrub speed, but it would be a shame for you to miss a clip along your way.
