How many times do you have to fall? I tried doing it before the achievements went live and gave up after 4 times.

You can fast travel back to the trial and complete this. No need to restart the game.

@Money Frag, if the achievment is bugged as I've seen a few reports of, then starting a new game would be required.

As stated above, you just need to fail during the “Trial of Hermès”. This can be achieved by falling to your death during a glide. This is the very first trial you face and is not really missable. Even if you complete the trial, you can always fast travel back to it later on. It is at the bottom of your map.
