In A New God, there are 24 story related trials, none of the trials are missable. Within each trial, there are 2 chests, one at the end and one secret hidden throughout the level.
Your goal is to open every chest on every level, as the hidden chest will hold one of 24 relics yiu need to unlock the secret trial.
Once you have found every relic (you can revisit levels if needed) place them on their chairs in the pantheon and speak to Hermes. He will then unlock the secret trial for you.
Enter the Trial, complete all 12 puzzles and leave the trial via the exit, and the achievement will pop.
EDIT:
As mentioned by AllOvaMyself, there is also a chest which gives you a new phosphor skin, located in "the room that unlocks after you complete the 12 puzzles that has a chest inside". This will also exit and complete the Dungeon, however doesn't relate to any achievements in the game, more for those who want to find everything.

When you say "leave the trial via the exit" are you referring to the one in the middle of the circle of chairs at the center of all the puzzles?
If, so, you missed the room that unlocks after you complete the 12 puzzles that has a chest inside. I wanna say it gives you a new skin for Phosphor...

To my knowledge there is only one exit for this trial, which is in the middle if memory serves.
While there is an additional chest, it is not required for the solution to the achievements, you do not need to open the chest to solve this. It also does not contribute to any other achievement in the game.

This DLC contains 24 main trials, each with a hidden chest in them. You need to find each hidden chest to unlock the 24 relics needed. Now, place them on the chairs in the Pantheon and talk to Hermes to unlock the secret trial. This trial consist of a bunch of minigames that reference classic games and real life sports. Finish the trial to earn this achievement / trophy.
