Just after crossing the river,you'll reach a cave with a ladder inside.Grab the ladder and put it at the right of the entrance of this cave to reach the icon.

Chapter 2 - Tihon
Immediately after the moving platform "puzzle" at the beginning of this chapter, you'll come to a dark cave. Inside, toward the end on your right, is a ladder you can pick up and move. Bring it back out of the cave, and immediately on your left as you're leaving the cave is a spot to place the ladder. Do so, climb up it, and go on the little ledge above the cave entrance to find an icon you can pick up up there, unlocking this achievement.
