Story related and cannot be missed. Once you get through the Iron Kingdom, this is the boss you face before opening the Chakra Gate.

Once you get to the end of your second visit to the Iron Kingdom, you'll have to battle the boss Moloch before you can unlock the Chakra gate. It's a very easy fight, although you may want some characters whose attacks can hit multiple targets at once. Simply break down Moloch's armor until you've exposed his weak point, which you'll only need to hit once. Once he's defeated, this trophy will pop.
