How to unlock the Madness at the Core of Time achievement in Infernax - Definitive Guide
To go to the future you need to get at least 90% complete in the Demonology. This will most likely need you to do good, pure good, evil and pure evil runs in order to reach all areas and see all enemies and bosses. But it seems like you can miss some as you will see a max of 95% in all 4 playthroughs and players are reporting that Gregor will do this once you are over 90% Demonology.
Once you are over 90%, get the Necronomicon again and go see Gregor in Tohan Passage and he will take you to the future.
In order to get the Necronomicon, you must be on an ultimate good playthrough doing enough good deeds to trigger the attack on the cult. (see Necronomicon achievement)

Last edit: Just confirming that you can use Game Wizard cheats and the Konami code to still unlock achievements and build up your Demonology for this ending. And if you're going to try to go for Ultimate Evil, then you really need to commit since it seems stricter than Ultimate Good as far as I can tell.
^CageDemon is from the choice you encounter outside of Arkos barracks at night, one screen right of the main town. Either option should get you the entry in your book so it's just a matter of showing up at night.
Possessed is pretty missable too: after you beat Stormheim Castle (3rd dungeon, east of Arkos) you have to go one screen west of Darsov Barracks/Cathedral. The priest Julius will be standing by the hole and will tell you about the situation. This is missable if you're teleporting and infinite jumping all across the map to speed through the 5 main dungeons.
I'm about to head into what I think is the Good dungeon and have 70% after doing just about everything I can think of, if that helps set expectations. I'm going to try an evil playthrough with game wizard codes before I proceed to see if that helps me go from Good to Ultimate Good and I'll edit in an update here unless someone beats me to it.
Edit: I sped through a cheat-filled evil run and it didn't add much to my demonology count, but I proceeded to the sealed door in my good playthrough and was surprised to find I had gotten mixed up. I was able to do the ultimate good playthrough after all and added quite a bit to my demonology. Up to 86% now and I guess I'm going to do another evil run, being more careful not to lock myself out of the last cult quest this time.

Also to note, you must be on a ultimate good playthrough to do this, so you can just load up your ultimate good run, you should get an autosave before you go into the last area

To travel to the future, you need to complete exactly 95% of demonology. Filled with an ideal passage for good + ideal for evil (you may also need to complete an ordinary passage for evil or good). When passing, 92% was enough. Complete completion was taken into account for true evil and good. (+ final choice for the bad guy - kill Robert. In order to get one boss in Demonology.)
