This achievement requires you to initiate fights with a perfect hit on enemies by holding and releasing 
(when your character flashes) for 20 times.
You will get 2 relevant achievements before this one for doing the same:
The Sneaky achievement in Infinite Guitars worth 17 pointsStart 3 Enemy Mech Battles with a Perfect Guitar Hit
The Stealthy achievement in Infinite Guitars worth 51 pointsStart 10 Enemy Mech Battles with a Perfect Guitar Hit
No stealth approaches are necessary as the above 2 claim though, as action part of the game doesn’t allow too much of sneaking, the best way to do it is to get in the way of enemies and hold, wait them to detect you and hit them before they hit you. Successful hits lead to a preemptive attack from you but not enemies. Do this 20 times and the achievement will pop.

The, Pants, Party
This method will be similar to the Overworld boss achievements described in Waste Disposal (50G), but done against the basic enemies roaming the dungeons. Simply whack them with a perfect guitar hit by holding 
for about half a second until your character flashes white, then release to intiate battle. If done correctly, the battle will start with you attacking rather than the enemy.
