The description is not great.You must PERFECT all levels,not just complete them all.So you must finish all 18 levels in the game with 10/10 Inops and 3/3 stars.You can do the 2 objectives in 2 runs if you want.

Was having a similar issue where this and the elec-city achievement wouldn't pop. When I beat the elec-city boss it would not save the mission progress of having saved the 10 inops. When I then closed the game and reopened it, it reverted to 7/10 saved. This issue repeated about 10 times.
The solution I found was to beat the mission with 10 inops, continue to the menu, reopen the same level, then quit to the menu, then close the game. When I reopened the game they both finally popped

I can't even complete 3-1.. The thing resets and the blade kills me every time.. It's either too slow or completely resets

Achievement requirements:
The Complete all Mine Levels. achievement in Inops worth 174 pointsYou must complete all the Mine Levels.
The Complete all Jungle Levels. achievement in Inops worth 227 pointsYou must complete all Jungle Levels.
The Complete all Elec-City Levels. achievement in Inops worth 236 pointsYou must complete all Elec-City Levels
For the final boss battle, check the video guide below by YouTube user Twisted Slippers Gaming (kudos to them!). If you have some gems by this point, you can simply keep using them to respawn, making this fight a piece of cake.
- Beat all 6 levels collecting all inops and all stars in ‘Mine’ world
- Beat all 6 levels collecting all inops and all stars in ‘Jungle’ world
- Beat all 6 levels collecting all inops and all stars in ‘Elec-city’ world
- Beat Bignus — the final boss — regardless of inops and stars collected
