To obtain this achievment you need to collect all armour upgrades for your spaceship along with all weapon upgrades for your ship as well.
Once you've collected all of the upgrades, the achievment will pop.

Try to avoid getting the last upgrade at the same time you get to 100% explored, only one of the achievement popped for me. Fortunately I didn`t trigger an autosave and was able to exit out so i could pick up the last upgrade again for the achievement.

Anybody have a solution to this glitching if it's already auto saved?

This will unlock on your way to unlocking Explorer. All upgrades are clearly in view and not easily missed.
