How to unlock the Ancestral Vision achievement in Inscryption - Definitive Guide
Eventually you will be introduced to the knife as an item that can be used during battle. Similarly to the pliers, the knife directly affects the scale by removing your eyeball and placing it on the opponent's side. After a battle where you used the knife, you will have to select a replacement eye from a box. To get the achievement you must pick the eye with glowing blue magical/sigil markings on it.
I didn't have the magical eye as an option until my second or third time using the knife, so don't be discouraged if you don't see it right away.

I am giving the four story related puzzle trophies in Act I the tag of Spoiler. Each of these deserve to be discovered in organic gameplay, but you may need a few pointers in the right direction. They are also connected as you can not accomplish the second trophy without completing the first and so on. Please read the walkthrough of these trophies with that in mind.
This is the third of four Spoiler-filled puzzle trophy walkthroughs:
Once you have completed the requirements for Uncage the Menagerie you will likely know what to do with your now freed Wolf Statue (*wink* it's to the right of the card puzzle cabinet *wink*) use this item correctly and you will be rewarded with a knife to add to your backpack. The knife works in the same way the Pliers work but the bodily sacrifice is a bit heavier then a tooth. The pain and impairment is only temporary. I do suggest that you use the knife towards the end of a battle though, because your opponent will offer a replacement only after the battle is over.
If you are moving quickly through these progression related puzzles, you may have obtained up to the freed Wolf Statue and the knife reward in your second or later play. To complete the next step, you would need to be in at least your third playthrough of Act I (whether you lose along the way or make it all the way to the end and defeat the final boss) for the proper item to be available
When offered a replacement, there are two choices that should stand out to you. To move the story along, if you can't pinpoint it, I suggest you pick the top left choice. The other abnormal item is mostly just for a quick laugh and doesn't help move the lore puzzles along, so pick the top left for a revealing look into the last part of the overall lore puzzle sequence (See Reborn Hope )
