How to unlock the Gruesome Encore achievement in Inscryption - Definitive Guide
This happens during Act 1 after some decent progression. You'll need three of the same card and visit the Mycologists twice on your journey. Combining 3 Packrat cards or 3 of the same pelt are good choices since they're easy to get multiples of. A Packrat card is given for free if you have 3 items in your inventory and visit a Backpack symbol on the map.
The Mycologist symbol on the map is a pair of mushrooms and, for me, they started appearing
If you don't have any duplicates still visit the Mycologists anyway since they'll give you a duplicate based on your deck.
If you're very lucky,
The Mycologist symbol on the map is a pair of mushrooms and, for me, they started appearing

I can confirm this can also be unlocked in Kaycee's Mod

Not sure why but I combined two pack rats and then a third the second time I found him but it didn't pop the achievement. I was worried I had to combine 4, 2 and 2 then combine those .

This achievement is much easier to do during Kaycee's Mod, as the mycologists and goobert options appear in the trail more often;
Goobert will try and duplicate any card you give him (+/- some attributes/stats) and the mycologists will do their hack-n-clash OP boost to any card.

This can be done during Act I or in Kaycee's Mod after you have finished the game. If focusing on getting this as soon as possible, please note that the Mycologists do not show up in Act I on the map until your third or fourth run, so don't worry if you have not seen any mushroom icons on your map during your first few attempts at the game.
The Mycologists take a card you have in duplicate and combine the cards into one adding the attack and hit points together to make a stronger, longer living card with any sigils that were on either card. For this trophy, you will need to have them combine two cards and then in a subsequent visit later, combine a third copy of the card to the overpowered card.
If you are struggling to get three of the same card, you may want to focus on visiting the Backpack icon after your pack is already full for a Pack Rat card. If you are thrifty with your backpack (or using the Pack Rat card during battles to refill your pack) you can get more than three Pack Rats in a playthrough.
If everything lines up for you in Act I to hit the Mycologists twice after having three Pack Rats or any other card, then you will get this trophy. If not, don't stress over it! Kaycee's Mod is filled with opportunities to do this. The Mycologists show up much less frequently in Act I then they do in Kaycee's Mod, so it may be better to take this on in the post-game.

This can be earned in Act 1 or in Kaycee's Mod. Most likely you will earn this in Kaycee's Mod because there will be multiple instances of the Mycologist, sometimes even in a row.
The Mycologist is a space you can land on in any of the maps. The Mycologist wants to merge two cards into one card. If you do not have two of the same card the Mycologist will pick three of your cards and let you pick one so you will have a duplicate for the next time you meet him. When they merge two of your cards the card you get has the combined power and health of both of the cards, as well as all the sigils the cards possessed.
To earn this achievement you will need 3 of the same card and to visit the Mycologist twice in the same run. Cards you can get easily are pack rats because you can one at each backpack if you are full of items. In Kaycee's Mod, you can get duplicate cards easily using the painter space.
Most likely this will not be something you get to accidentally. You need to focus on landing on each of the Mycologist spaces. If you have a combined card and land on a painter space make sure you duplciate the combined card.
In Kaycee's Mod, once you unlock the Mantis starter deck, you start with two ring worms. If you quit out of a match twice in a row you will start with a third ring worm. It doesn't help your deck any, but you will have 3 ring worms. Now you just need to stay alive long enough to get to two Mycologist spaces.

When you have a copy of a card in the deck, go to the mycologists (mushrooms on the card). After that, look for a third such card and go to them again.
