Don't be confused by the name of this achievement. This actually has nothing to do with the score but rather the multiplier that you earn during the level. Although the Achievement picture shows "x999", the multiplier required is actually a lot less. While I cant be 100% sure what combo is required, this unlocked for me right after I lost a combo of 256, so if i had to guess, id say you aiming for around the 250 mark.
While this may be possible by simply completing the levels without dying, you will greatly increase your odds of earning a high multiplier by pressing the B button to the music tempo when you do not need to use any of the other abilities. Each action gives you +1 multiplier, so it can be achieved long before the end of a level if you are pressing B at every available opportunity. As I mentioned above, the achievement unlocks once you lose the multiplier.
