You can earn this as soon as your third Key Rune purchase.
Once you purchase an additional key rune you can begin to combine them to get a "different" run which typically doesn't do much besides combine the enemies from both runes into one environment.
You'll have to go through all of the combinations for the 45 villager achievement so it doesn't really matter which 5 combinations you do to earn this one, easiest would probably be doing each of the three keys individually and then combining The Obsidian Fortress with the other 2.
Don't forget to choose villager rooms at Depth 2 and Depth 4 and ring the bell before ending their stage!

Is this possibly glitched? I completed the first dungeon a couple of times and the fungal one, but it shows as if I completed only one (instead of two). Got all villagers on both too.
