As the description says, kill all enemies in a room without picking up a weapon. This achievement will possibly happen naturally, but it's incredibly straightforward to do right at the start of a new run, in the first combat room. Enemies just walk around and don't shoot or anything, so just finish them off and leave to unlock this achievement.
