These can either spawn randomly in a room, or from red pots.
I've seen them in Floors 3, 4, 5, and 6. (Floors 5 and 6 are unlocked by beating the regular New Game Campaign). They'll also spawn in some of the later rounds in Endless Mode around/after Wave 10.
They're fairly annoying to kill, even with upgrades. They move pretty quickly, take more damage (Comparatively to the regular enemies, that is. I believe it's anywhere from 4-6 hits depending on your upgrades), and leave a lot of fire spots behind that'll hurt you if touched. The achievement pops almost immediately after you kill just one of them.

Fire Bugs are enemies that can randomly spawn in pretty much any (non-boss) combat room. In my experience, they pop out of jars or barrels when you destroy them. They'll be pretty obvious, since they move decently fast and leave fires in their wake. They take easily five shots of you default weapon to destroy, so it can be tough if they spawn while you're still in the thick of many enemies, but if you can focus on one it's pretty easy to destroy. You'll see a handful spawn over your time with this game, so just make sure to kill one of them, since there isn't anything you can really do to force one to spawn if you need one.
