The Ledger is available from the main menu.
It consists of pages of any encountered/unlocked items from the game and gives often helpful descriptions of each.
Once you've played enough of the game to have encountered 20 different items (such as chests, weapon power ups, food, other score items, and scrolls), then navigate to the Ledger from the main menu.
Press
on an item to view it's description and progress the achievement. Use

From the main menu, you can access The Ledger. This gives you an explanation of what each item does (if it actually does something). Every single thing you pick up in the game gets added to the Ledger, and there are over 200 entries in it. You will have 20 within the first hour of playing this game. Simply open the Ledger at any point and view descriptions for 20 different items to unlock this achievement.
