This is an item drop that occurs during gameplay.
If you get hit twice in a row, the Golden Poop item will drop worth 5000 points.
Pick it up to unlock the achievement. It will likely disappear after a set time like most items.
This can probably drop on any difficulty or room, and definitely drops on both Normal and Easy.

The Golden Poop is a drop that isn't entirely random. I believe what you need to do is get hit twice in a row at least, without killing any enemies in between getting hit (in fact, don't even shoot any), and then kill the enemies after getting hit at least twice. This should trigger a Golden Poop to drop, which just gives you some points, and this achievement of course.
