As the description says you must upgrade your stats at least 5 times during one game (in other words without getting a Game Over screen)
In order to get Upgrades, you need to either chain 8 Food Pickups together or buy them in the shop, however the shop usually only have 1 or 2.
The different stat increases are:
Extra Damage, Speed, Extra Fire Rate, & finally Extra Weapon Duration.
FOOD DROPS: (Info)
Food drops, if not always then at least, most of the times when you kill an enemy after that you have a certain amount of time to collect it before it disappear, once you have collected you 1st Food drop you only have a small window to get another piece of food in order to get x2, x3, x4 & so forth ... Now I'm not 100% sure that a Stat Upgrade (or anything for that matter) drop everytime you get to the x8, I am however 100% sure that they drop sometimes when you get the x8.

This is essentially mandatory to complete a run. As the tips and advice details in the roadmap, your success depends almost entirely on your ability to get stat upgrades, and you'll be getting somewhere closer to ten on your New Game run, unlocking this achievement along the way. Refer to the roadmap for more details on getting stat increases.
