This is a story related achievement and cannot be missed. You should get it within an hour or two of starting the game unless you're taking things very slowly.
Once you leave the dungeon/sewer tutorial level you need to clear the "Sewer Exit" map which then gives you access to the strategic island overview. But there's no option other than entering the "Clearing" map where you meet a lone boy who tells you about an aid station up ahead. Exit this map and enter the map called "School". Kill the enemies and talk to the aid worker named Veronique to earn this achievement and open up more of the island.
