This is not that hard, but there are a couple of missions that you will need to focus on.
The Prison. You will need to stealth all the way around the prison and snipe the Black Merc before setting off the alarm. He is the only one that executes prisoners - so if you get him first, the prisoners will be safe - and you can clean up the other guards from the back of the prison. Easiest way I have found is to cut the fence and head to the left. A few stealth kills will get you all the way to the back wall, behind the prisoners cages, where you can crossbow the black merc in one shot.
The other area I slipped up was in the research lab with 4x super soldiers. If you get grenade crazy you will kill the scientist and that will cost you the cheevo. You can pretty easily split your party and make them run (the dont have many action points) so you stay away from them and shoot them from a distance.
Otherwise, just watch the levels that have civilians when throwing grenades or using Full auto Rage Machine guns etc,,,,
