During the section of the level where you are driving after the train simply keep the right trigger button held down completely the whole time. Do not press the left trigger button. You will go through 3 sections of the escape and must do this for all three sections.

Dont worry if you die on the ice sections you will still get the achievement if you retry these sections after you have died just make sure the right trigger is held down completly on every try. (this will work when you select retry from checkpoint, just make sure you keep the trigger held down all the time - even during the loadiing screens - credit to Calex dEUS for checking that)

The achievement will pop after the third driving section when the cut scene starts.

...and to add to trufflesonline and DRKstar47 suggestions. Guess what !? No brakes, no problem, you can still use your emergency brake !. As they mentioned before, just never let go of the the gas.

I even hit a couple trucks and spun out but I held on to that right trigger for dear life and still got it.

Hold the rt (throttle) at all times, even during the cutscenes/loading
1st section is easy 2nd section is easy 3rd section is a little tricky but with a little luck you will remember the path -
i didnt even make the final jump and my achievement unlocked lol

Just to add to trufflesonline's guide. You can use the walls and rock faces to clow your car down if you are haveing trouble with some of the sharp turns. It allows you to be at full throttle but still slow down enough to react to the sudden turns.
