The achievement asks you to perform a chain of three Focus Aim kills. This is easy to achieve. This is how I worked on it.
Go to the Monaco section and work your way through the garden. As you do so, you can earn three focus aim kills easily. Just do silent take downs as much as you can and don't do any shooting until you reach the room with the Security Chief. He'll be surrounded by his bodyguards and it's your job to take them out. There will be one guy going up and down, the Security Chief in the back, along with one more guard. Once the guy, the one that is walking back and forth, has his back turned on you, immediately use your Focus Kills on all three bodyguards.
All of them should go down fast and the achievement should unlock for you. Have fun and good luck!

In Athens, when you reach the parking lot where you can get No Valet Required, you can also get Three Birds, One Blood Stone by waiting for the red BMW that slides in to stop and unload two henchmen, while one is already present and shooting at you in the parking lot.
Once the two henchmen are out of the car, start making Focus Kills, and voila, Achievement Unlocked!

A good place to do this is in mission three (Monaco). After using stealth takedowns on the first few enemies, you should have three focus aims. Then just perform focus kills on the next three enemies. They do not have to be in quick succession, as long as you do not fire your gun or use a takedown inbetween them.
