It doesn't seem possible to get them all in 1 playthrough of the act. You unlock codex entries as you do specific things, or meet specific enemies. I was missing 1 entry by the end of the act, so on my second run through I chose different options and got the last entry I needed.
The entry I was missing was the Giak Language. For that I had to charge into a fight when Leandra appeared.
I can't give specifics on how to unlock each entry as it doesn't notify you as you play. You can see what you have by opening your journal (Press
and select third entry at the top, a notepad and quill) and pick the second option in the top right (A monsters face) or from the main menu under the Extras > Codex section.
