Location: Studio 51 boss
To clarify (or correct?) the guide above, you can die. I believe the key to unlocking this achievement is to not let the “glitched” sequence automatically happen. You need to cause the glitch sequence to happen by successfully hitting him 4 times without dying in between. I was having a hard time rolling and getting a hit on him so I decided to try jumping over him and hanging on the wall so he would run into it. After he runs into the wall, jump behind him and you shouldn’t have any issue getting a hit in.

Hey, figure I should add: If, and when, you fail to do this right on your first try, just dashboard out as soon as the screen starts to glitch. Then reboot the game and continue, keep at it until you get it.

Great solution. Can basically cheese the fight by sliding on the wall.

This achievement requires you to beat the boss at the end of the Studio 51 level (Level 5) without getting hit once, this includes dying and resetting. However if you do die, you can return to the main menu and reselect the mission and you'll start just before this fight. As for tips in the fight itself, take your time and make sure to roll 
behind his attacks, once you've gotten 4 backstabs in the fight will end, and the achievement will be yours.

I unlocked this today, despite dying earlier and having to reset. I'm not sure if I was hit during the last fight or not though.
