On the Bunker level (Level 10) after going down a few floors, the elevator will open on the right hand side next to two enemies with a molotov on the console, pick this up and then clear the floor without using it. When you step into the left hand elevator throw the Molotov 
at the wall, it will kill you and you'll receive this.
