This bonus level can be unlocked after finding and collecting all bonus (green) crystals in the fourth world.
This level is an elevator ride through a pure bullet hell, and is definitely the toughest of the bonus levels. You need to move around on an ascending elevator to avoid bullets shooting out of the sides all over the place. Thankfully, like all levels, there is no actual randomness, so it's just a matter of learning the patterns. The first portion is relatively easy (just stay in the middle as much as possible and jump when needed); the second portion requires some back and forth across the elevator; the third portion just feels like all-out chaos. This one will definitely take a bunch of attempts to beat, let alone doing it without getting hit. Definitely watch the video in Wait... What? (100G) as needed.
