To obtain you must beat the game with every character on normal. The achievement doesn't stack if you beat the game on difficult so it's really up to you whether you wanna start on normal or difficult first.
The best advice I can think of is to upgrade your attack to a 3-hit combo, preferably an easy one such as Y-Y-Y or B-B-B. You cannot do this for every character however so try for one like Y-Y-B or B-B-Y something kinda easy.
Secondly when doing a 3-hit combo you can press the guard button to cancel the combo. A tactic I used was to do a 3-hit hit combo and as the 2nd hit was landing cancel it out and start again. With this you have almost no time to be vulnerable but you stamina will completely drain and stay drained, so be wary when fighting bosses.

Buy a power move the first chance you get, when fighting a boss, make some distance (don't lock on), do the moves that lead to the power move, charge the power move, at the last moment lock on to the boss (as in just before boss is close enough to attack you), then release the power move- it homes in.
It sometimes takes 2 or 3 of these powered slashes depending on how much charge you get to do, so soon as it hits, take lock off then make some space again between u and the boss.
