Your Defense rate must be 80% and you must have blocked atleast 50 attacks in a stage to get this.

For this I just stopped at the beginning of a level and got into a corner where enemies couldn't attack me from behind and stayed there for about ten minutes. That way I'd have more than 50 blocks and in case I did bad at the end of the level my defense rate would still be above 80%.
