Here's my video walkthrough of Act 3, complete with this achievement:
1. When on the Zero, find the bureau (if you wait, you can click the ZZZ in the bottom left).
2. When you find the Bureau, drive counter clockwise (about two laps for me).
3. You'll reach the TV. Stop, slowly move the other direction (clockwise) and you'll see The Van. Go here.
4. As the dog, walk to the far left of the van. You'll see Phil and Jane up on the hill.
5. Achievement unlocked! Feel free to leave!

I thought I was in the wrong spot when I entered the Van, so I immediately backed out to the zero and didn't do anything with the dog or interact with Phil and Jane. I couldn't get back to the van, so I thought I was screwed. Achievement popped at the end of the chapter. Whew!

In act 3 when reaching the zero near the bureau, go counterclockwise to the TV, Then turn around you we'll find a van click on it and you well be taking control of the dog walk towards the van and this achievement should pop.

Act III in between Hall of the Mountain King and revisiting the Bureau
When you leave the Hall of the Mountain King, head around Route Zero until you find the Bureau of Reclaimed Spaces but do not get off the Zero yet.
Now head counter-clockwise until you find the TV (for me it was three full rotations around the Zero). Linger at the TV landmark until the option to take the exit for 70 appears. Pick this route, head up slightly to the Van (the icon looks like a diamond) and go to that destination.
This will start a scene with just the old dog, walk it over to the van to the left and you can see two people standing on the hill far left. The trophy should pop once you can see the figures on the hill.

After the Hall of the Mountain King, in the Zero, go counter clockwise to the tunnel, choose the route, then head clockwise until you reach the Bureau, then head counter clockwise until you get to the tv, and stop here. Slowly go back the way you came until the van option shows, and enter. As the dog, walk to the far left to unlock.
