How to unlock the Failure is Not an Option achievement in Kerbal Space Program - Definitive Guide
This achievement will not pop when you land on Moho . The achievement does require you to land on Moho, but it has a hidden requirement that you successfully complete your mission, thus it will pop when you land back on Kerbin.
In terms of building a sufficient rocket and the most efficient way to overcome the sheer amount of Delta V required to complete this mission, I found this thread useful:

Bug Alert: I'm not sure if my rocket had a flaw in the design or that there was a bit of lag between when heat was calculated and the radiators kicked in, but for safety sake, make a save when you are passing through the orbit of Eve. My rocket would explode whenever I tried to reload a save closer to the sun, so I don't know if I was being instantly cooked or some part was interacting with another and because of the speed involved I got torn apart as the ship loaded.
Another issue, after you make your insertion burn and leave Kerbin, don't fast warp as you leave Kerbin's sphere of influence. For whatever reason, the game would think I made another encounter with Kerbin or one of the moons and it would completely alter my orbit. I only had this issue when heading to Moho, never another planet.
