You need to unlock the EVA ability in the science tree first if you're in career mode.
During an orbital or suborbital flight above 70k meters, put the cursor over the picture of the Kerbal and click EVA, I'd advise not letting go of the ladder unless you know what you're doing. Hold B for more than 1 second (make sure you're not in cursor mode, click left stick) to climb back in.

Did this unlock for people for just being outside the craft? I've tried this twice and it didn't unlock.
*edit* This finally unlocked for me in fairly deep space. My original attempts must have been too close to Kerbol.

Can also use a stock craft in sandbox mode,so no need to unlock in science/career
