In order to unlock this achievement, you need to escape from the exit the icecream truck unblocks during the Klownpocalypse, which occurs during the last minute of the game or if the klowns plug in all 16 cocoons (icecream truck doesnt spawn).

Just to add, if the Klowns plug in all 16 cocoons the ice cream truck will not spawn during the Klownpocalypse.
