This will be your very first achievement in the game and it is unlocked just after you started the game for the first time.
When you load up the game for the first time you have to go through some guided tutorial that involves having your face scanned for your virtual avatar. After the face scan you will see a first version of your avatar that can be customized in the following screen. Adjust everything to your personal preferences and choose to create your avatar. The achievement should unlock as soon as you are done with it.
