After you pass the section where you see the giant whell that keeps the Dragon chained up, you'll descend down a ladder to arrive on the same level as the Dragon.
Cross the long path ahead over or around the piles of bones and you will arrive at a fork in the road. Common sense would tell you to turn left away from the Dragon, however you can stay straight and walk right up to him for a funny scene and this trophy.
Note: the trophy will pop AFTER your "death" and the ensuing short conversation with Gwendolyn about seeing if she was paying attention. I didn't hit Continue until after the trophy popped.

In the prologue, you will come across a dragon sleeping on one side of a hallway with bones scattered on the floor. After stepping on the first set of bones, you will have to run to a bed and hide from the dragon’s sight. Once it goes back to sleep, avoid the bones ahead of you a bed will fall and the dragon will wake up again. Hide in the bed. Once it goes back to sleep, walk up to the dragon; it will wake up and eat you; killing you.
