This took awhile to figure out. You must be tired when you wait for the 2 days.
In short you will never get this achievement if you boost your energy above 50. It needs to be below 50 and then you can use the wait timer.
It took about 5 in game days of just exploring waiting for the time to pass just for me to find out that the achievement isn't progressing because I was keeping my energy high.

The description for this achievement is incredibly misleading. The actual unlock condition is Have 50 energy or less for a total of 48 in-game hours (does not have to be in one sitting).
This means that achievement progress is cumulative, just like the other "starvation" and "stuffed" achievements. This ALSO means that if you have 50 energy or below and are sleeping to raise your energy, achievement progress will still be tracked even though you're sleeping.
So, simple enough, keep your energy below 50, spend some time reading books, doing quests in towns, stuff that doesn't require you to have a lot of energy. Don't be afraid to go sleep when energy drops low, on the contrary it's a great way to pass time for the achievement.

Ugh, sadly, you can't just "Wait" two days like I tried. No idea why.

I got this earlier even after sleeping a bit. My sleep bar was almost completely full and my eyes kept shutting so I laid down on a bed and slept for four hours and it raised my energy from 13 to 55 which still kept my bar almost full. After that I ran around town doing quest for a few minutes and the achievement unlocked when my bar was 1/3 of the way filled up.
Edit- Forgot to mention that I don't have Insomniac and I never drank or ate anything other than one piece of cooked hare meat and that was when my sleep meter hadn't even shown up yet.

Don't sleep and you'll eventually get the debuff "Tiredness". You'll probably get this naturally throughout the game by just not paying attention but if you want to rush it, you can just keep waiting a day at a time until it pops.
If you want to do this achievement quickly, press the center pad to skip time. You can only skip 24 hours at a time so go ahead and do it twice. Make sure to have food ready to consume since your nourishment stat will drop during the time skip.
Supposedly, waiting around won't work unless you're already tired, so...
Wait around/play the game until you have the tiredness debuff, indicated by the bed symbol. Then press T to wait for a whole 24 hours. I recommend you buy a room first just to be safe. Do this as many times as needed to get the achievement. Don't worry about your energy dropping to zero. Nothing happened to be, but you usually just pass out, and there's no apparent negative effects of that.
