How to unlock the Plague Doctor achievement in Kingdom Come: Deliverance - Definitive Guide
This quest is timed, so if you don't heal the villagers in time you need to restart from a save at the beginning of the quest.
You need be able to know how to read to heal all the villagers, so do that at the scribe in Uzhitz before travelling to Merhojed.
You'll also need charcoal, valerian and thistle to prepare the cure.
Now might be a good time to make a manual save.
Since you've made a save, you could try to finish the quest yourself, but if you want the full solution check out the spoiler tag:
Go to the village, spoke with Melichar. Go speak to to Mattias in the stable. After that go to speak with sick people, to get grip on their symptoms.
List of sick people you have to speak with:
Talk to Bedrishka, dig her husband a grave and bury him
Talk to Daniel
Talk to Vincent
Talk to Old Straw
After you finish speaking to them, go to Sasau monastery to speak with brother Nicodemus. On the way buy from any blacksmith some charcoal. (you will need it for alchemy recept later) Speak with Johanka and go tell Brother Nicodemus about the plague and tell him that half of the houses and all of the animals have fallen ill. Then say “I know” and “I can read”. Your other conversation options doesnt matter. He will tell you about the book you have to read, while he go to read another book, but you Speak with him before reading it and buy some Valerian and atleast 2 Thistles from him.
Go read the book of the lectern nearest the door. Tell Nicodemus that you “know what ails them”, and say “poisoning from water”
Say “I can help” and offer to brew the potion yourself, otherwise villagers will die
After that, brew the healing potion and take the cure to Merhojed and talk to Nicodemus.
Give cure to all the sick people – Straw, Vincent, Daniel and Bedrishka
Talk to Melichar and ask to see the captured bandit
Cure, wait for and then interrogate the prisoner
Talk the villagers down when they come to attack the bandit
Wait a couple of days for the situation to improve and speak to Brother Nicodemus.
How to brew the Merhojed potion:
head to an Alchemy Station – there’s one conveniently placed next to the infirmary, or behind the apothecary in Rattay.
This step is complicated and can go wrong, so familiarise yourself with the steps first so you don’t get flustered and mess up. It’s a good idea to drink a Saviour Schnapps here just in case.
First, use the Alchemy Station and open your inventory with the d-pad, placing the Valerian, Thistles and charcoal on the bench.
Next, add water to the cauldron. It’s yellow and on the shelf to your top left.
Then add two portions of Thistle to the cauldron. Don’t mash them up in the pestle, just pop them straight in. This makes the mixture turn a funny grass texture.
Now you have to boil the concoction for “two turns”. You turn on heat by pulsing the bellows with the left trigger. Press the button 4-5 times until you see black smokey bubbles in the mixture, this means it’s boiling.
When you see the bubbles, press the right trigger to turn the hourglass. And when the first lot of sand runs out, press the right trigger again to flip it over.
Now that the sand has run out for a second time, add the Valerian from the bottom right shelf.
You now have to boil the potion for one more turn. So press the left trigger until you see the bubbles again. Go slowly, because it’s possible to overboil the mixture. Stop pressing as soon as you see the bubbles.
As the bubbles start, press the right trigger to turn the hourglass again.
After the sand runs out, wait for the flames under the cauldron to stop, then add the charcoal from the bottom right shelf.
Then take a phial from the bottom left and decant the potion.
You should get a message on-screen that says you’ve brewed Aid for Merhojed.
To sum up, the recipe is:
Add water to cauldron
Add two handfuls of Thistle
Boil the mixture for two turns of the hourglass by pressing the left trigger 4 to 5 times.
Add the Valerian and boil for another turn
Wait for the flames to go out
Add the charcoal and pour into phial.
It's his wife that needs to be cured, felt rather dumb when i realised that.
MISSABLE
This takes place during the main quest "Questions and Answers".
You'll obtain a side quest during this called "Pestilence" where you'll be tasked with healing all the sick in Merhojed. This is a TIME SENSITIVE quest so make sure you start it as soon as you get it. Before starting the quest, to make things a bit easier, I'd have on you Charcoal (x1), Thistle (x2) and Valerian (x1) as this is what is going to be needed to make the cure. Also, make sure you are able to read, otherwise you run the risk of wasting too much time and have villagers die.
Go talk with the Bailiff in Merhojed and agree to help. Go to the 5 houses marked with a white cross marked on the door and talk to the people inside. Once finished, it'll mark the objective completed, then head to the monastery to talk to Johanka. She'll then have you talk with Nicodemus.
Pick the options when talking with him: "About half the houses", "All the animals died", "I know", "I can read", "Where to we start?". Read the book marked with the objective maker and flip through the pages. Then go back and talk with Nicodemus and tell him you know what ails them. Tell him it's "Poisoning from water". Then make sure to pick the option that "I can help", otherwise he'll do it and it'll take a day and villagers will die. The last dialogue options don't matter.
Turn around and use the alchemy station to make the cure. If you don't have the ingredients, there are some that are able to be stolen out of a sack left of the station. You might want to save before you try to make the cure, just as a precaution in case you screw up.
Place your ingredients and add water to the cauldron. Then add two handfuls of thistle and boil for two turns. Then add the valerian and boil for another turn. Once done boiling, add the charcoal and grab a phial and complete the potion. Now head back to Merhojed and talk to Nicodemus.
Go back and heal all the people you talked to; Bedrishka, Daniel, Vincent and then talk to the Bailiff. He'll allow you to see the prisoner, who you'll need to heal as well. After healing all four, you'll have to wait a few days and go back and talk with Melichar.

There are several diseases in the book, but some of them are inappropriate because animals do not get it, others indicate a rash or wild dancing. As a result, it turns out that the most suitable option is water poisoning. Air poisoning, for example, causes a rash, but dysentery is not accompanied by vomiting and does not affect animals.
Agree to prepare the potion or let Brother Nicodemus do it (then you will have to wait a day). The Mrhoed medicine, with a correctly indicated diagnosis (water poisoning), requires one handful of coal, one handful of valerian and two handfuls of thistle. First add water and throw in two handfuls of thistles. Boil for two turns, then add valerian and boil for another turn. Wait for the water in the cauldron to cool, then throw in the coal and pour the potion into the flask on the left.
Jump to Mrhoedy and talk to Brother Nikodim, marked with a marker. You will have four servings of the medicine. Give them to Danek on the western outskirts of the village, Chenek on the eastern edge, Berdyshka, whose husband you buried earlier, and the robber. To get to the latter, talk to Melihar and say that you helped the village. Go to the robber and give him the medicine. But that's not all. For some reason, you need to interact with Soloma's wife, who is being examined by Brother Nicodemus. You will give her the last medicine - the objectives of the task will be updated and an inscription will appear saying that you need to wait for the situation to become clear (when the medicine works, if you have correctly diagnosed the disease).
You will need to wait about three to four hours for the targets to update and for the medicine to take effect. Return to the robber and find out information. When Straw breaks in, decide what to do. You can persuade him not to kill the robber by saying that he should not become a murderer. This will allow you to automatically travel to Rattay and put the robber in the dungeon.
You will also need to return to Mrhoedy in two or three days and talk with Melihar. If you made the correct diagnosis and rewarded everyone with a medicine (for water poisoning), then Melichar will thank Henry and offer a reward.
